Application Fee: $25/per child by June 1, 2010 and $50/per child after June 1, 2010

Tuition Payment Plan Election – Please choose the preferred payment plan below:
PLAN A – I (We) wish to pay the full year’s tuition obligation (excluding registration, books, and basic supplies fees) of $4,275.00 on or before August 1, 2010 (a 5% discount is applied for tuition paid in full that is RECEIVED IN SALMA K. FARID ACADEMY on or before August 1, 2010).
PLAN B – I (We) wish to pay semi annually and will pay the first installment of the tuition and fee obligations (excluding registration, books, and basic supplies fees) of $2,250.00 on or before August 1, 2010 and will pay the second installment of $2,250.00 on or before December 1, 2010.
PLAN C – I (We) with to enroll in the Monthly Tuition Payment Plan and understand that I (we) must pay the monthly tuition fees directly to Salma K. Farid Academy. I understand that if I choose this option, I will be responsible to pay a monthly tuition of $475.00 on the 1st of each month, 1st payment being due on August 1, 2010.
*A 10% discount (tuition only) is applied for all additional siblings enrolling in the Academy, after the 1st child. Non-Refundable Registration Fee due with submission of Application, and Application Fee: Non-Refundable Book & Basic Supplies Fees due by August 1, 2010.
